Tribal Health Plans, COVID and CARES ACT

By Karen Knight & Tyler Moore

How the COVID pandemic is impacting health insurance plans for Native organizations, including how to evaluate current claims, project future costs, factor in treatment and diagnosis expenses, and position tribal health clinics as cost-management assets.

When COVID-19 arrived, tribal health plans faced a set of financial and operational pressures that were both immediate and structurally novel. On the cost side, the pandemic created significant uncertainty about future utilization, including whether deferred elective care would surge back, how COVID treatment costs would flow through self-funded plans, and how stop-loss arrangements would respond. On the revenue side, CARES Act provisions created new opportunities that most tribal health plans were not positioned to access quickly.

This article, by Karen Knight and Tyler Moore, addresses both sides of the equation. On costs: how to read current claims data to understand COVID-related utilization patterns, what to model for future claim costs, and how to evaluate whether stop-loss coverage is structured appropriately for a high-uncertainty environment. On revenue: which CARES Act provisions apply to tribal health plans specifically and what steps are required to access them.

The article also covers the role of tribal health clinics as cost-management assets during the pandemic, specifically how clinics operating under IHS or FQHC structures were able to access COVID relief funding that self-funded plans without a clinic component could not. For tribal employers without a clinic, this created a clear and urgent case for clinic development.

Written in August 2020 at the height of early-pandemic uncertainty, this piece reflects a moment when tribal health administrators needed practical guidance quickly. The frameworks it provides for evaluating plan performance and positioning for CARES Act access remain relevant for understanding how tribal health plans responded and what structural advantages proved decisive.
